About Us

Established in 2006, Grace Hospice is a Community Health Accreditation Partner (CHAP) accredited hospice agency providing patient and family centered care that optimizes quality of life. Our care team works together focusing on the patient’s needs in their home, home of a loved one, or in a long term care or assisted living facility.

Grace Hospice is a member of the National Hospice and Palliative Care Organization (NHPCO). In addition, we are national partners of We Honor Veterans, a pioneering campaign developed by National Hospice and Palliative Care Organization in collaboration with the Department of Veterans Affairs.

Grace Hospice is a division of U.S. Medical Management (USMM). USMM is a continuum of companies and a premier provider of home-based medical services for elderly and other adult patients with complex health issues.
